Case Management services

Community Long Term Care (CLTC) offers programs to help individuals who want to live at home, need assistance with their care, and are financially eligible for Medicaid. 

The objective of case management is to counsel clients regarding services and support. Case managers assist participants in gaining access to needed waiver and other State plan services, as well as medical, social, educational, and other services, regardless  of the funding source for the services to which access is gained. The case manager also assists clients in coping with changing needs and in making decisions regarding long term care. He or she also ensures continued access to appropriate and available services.
